Matt Smith leaving?

I think after Ian (?) called him "Doctor Foreman", and he responded "Doctor Who?" he probably decided that he quite liked being called that, and thereafter used it on occasion even though, following Time Lord tradition, he had originally been known as the Doctor. (How WOTAN got hold of it is anyone's guess.)

Now for something simpler, like whether Aristotle or Plato was right about the fundamental nature of reality.

He was great to start with, surprisingly old and alien (I had no hopes whatsoever for him so was pleasantly surprised) - but like the two before him, he was badly let down by the writing.

A good point Liz, I was quite moved by the Vincent episode, beyond that I'm afraid I found Smith too young for the role and poorly portrayed, lacking the presence required to do justice to the part.
